Pursuant to the public records law, I would like to request the following records in electronic format sent to my email address.
The UIPA Record Request Log(s) used by the Department to track record requests received from July 1, 2023 - June 30, 2024 for the following entities:
(1) Maui District Health Office;
(2) Office Planning, Policy and Program Development;
(3) Solid & Hazardous Waste Branch (only need logs for July 1, 2023 - December 31, 2023 for SHWB);
(4) Wastewater Branch;
(5) Developmental Disabilities Division (only need logs for July 1, 2023 - December 31, 2023 for DDD);

Aloha Jon, I was looking for the Excel spreadsheet that the Wastewater Branch would have completed for UIPA requests. In the past, the Branch has never had more than 31 such requests (in FY2021) and usually much less than that.

Aloha Jon, since there was only one UIPA request to the Wastewater Branch in FY 2024, I would like a copy of the following documents: (1) AECOM's request; (2) the Branch's acknowledgement to requester requesting additional time, if any; (3) the Branch's notice to requester with any estimate of fees and costs; and (4) the Branch's transmittal correspondence with final disclosure of the records requested by AECOM.
