Aloha Lois, I think there may be a different log--specifically for the first half of FY 2017--because the information uploaded to the semi-annual master UIPA log on data.hawaii.gov shows that the Department had one request in the first half of FY 2017, while this log shows no requests. See Row 41 at https://data.hawaii.gov/Government-Wi....
I am looking for Excel spreadsheet that has a record of that request from the first half of FY 2017.

Hi Brian,
I was able to get clarification on the matter and provide the attached Excel spreadsheet which shows -0- for the period 7/1/16-12/31/16. This Excel spreadsheet was submitted along with the "checklist" in 1/2017 - this checklist estimated that there were "1" inquiry per year. That was an estimate only, and the actual Excel spreadsheet submitted simultaneously showed -0-.
Thank you for the time to research this. I assumed this reporting responsibility this past July and there were two others doing the reporting in the year prior. Please let me know if you have any further questions. Thank you.
