Aloha, thank you for the note about OEQC. There were a couple of files that did not sufficiently identify the relevant agency. There was a file labeled as OPHP and one labeled as OMCCR; if you let me know which agencies those are, that may address two of the issues identified below. There was a file in a different format as well, which did not have a descriptive label to identify the agency ("UIPA Log 1.2022-6.2022.xlsx") and if identified may help with the issues below. It appears that the following files are still missing. Hi Brian, * OPHP is the Office of Public Health Preparedness * OMCCR is the Office of Medical Cannabis Control and Regulation * The State Hospital is included in the Adult Mental Health Division * The Communications Office is included in the Director's Office (DO) * The Environmental Management Division logs are all of the logs that start with EMD * The Disease Investigation Branch is the unlabeled excel spreadsheet. * The Family Health Services Division offices are the logs that begin with FHSD. * The offices that did not include semi-annual logs reported that they did not receive UIPA requests for those time periods. * I am confirming that HIPAA, HRO, OHCA, and WWB did not receive any UIPA requests for FY22 and will let you know as soon as I am able to confirm. ________________________________ From: R. Mahalo nui for the explanation and for continuing to follow up with HIPAA, HRO, OHCA, and WWB, Kaitlin. In prior years, State Hospital, Communication Office, EMD, and FHSD had their own logs, but understand if those got consolidated because the numbers tended to be low (if any at all). Given historical numbers, most of this makes sense. But I would ask that you please check with Immunization Branch, Safe Drinking Water Branch, and HEER. It would be unusual if Immunization Branch had 0 requests all year. Given all the activity with Red Hill in the last year, I thought Safe Drinking Water Branch would have at least a few requests. And HEER typically has a relatively higher volume all year, so no requests in the first half of FY 2022 seems strange. Lastly, I apologize; I missed that CAMHD is only the second half of FY 2022. Given its relative volume, it would be strange that it had no requests July - December 2021. Best, Brian

Hi Brian, Got this response from HIPAA: The HIPAA Office does not make UIPA disclosures. All disclosures of HIPAA protected health information (PHI) (whether for access by the individual or disclosures for treatment, payment, or health care operations) are made at the division level. The divisions that are subject to HIPAA are ADAD, AMHD, CAMHD, DDD, EMSIPSB, and the OHSM business associate section for the EUTF. If the Civil Beat is requesting HIPAA PHI disclosure logs, it can only be disclosed under consent from each individual. Otherwise, it is protected under federal law (both HIPAA and 42 CFR Part 2) and I’m not aware of any exceptions that allow disclosure to news media. Can you check to see if you need any of the UIPA logs from the following offices: - Alcohol & Drug Abuse (ADD) - Adult Mental Health Division (AMHD) - Child & Adolescent Mental Health Division (CAMHD) - Developmental Disabilities Division (DDD) - Emergency Medical Sevices & Injury Prevention System Branch (EMSIPSB) - Office of Health Status Monitoring (OHSM) Thank you, Devin

Aloha Shawn, I am following up on my November 17, 2022 request for DOH's UIPA logs--see my March 31, 2023 e-mail to you with the outstanding portions of the request. Devin resolved most of the issues, but she was double-checking with Immunization Branch. I sent a request for update today and received an automated message that Devin is no longer with DOH. The remaining issue with Immunization Branch: Kaitlin indicated 0 requests all year, but historically Immunization Branch has had from 3 to 13 requests per year for the last several years, so 0 requests seemed strange. Devin indicated that she started following up with Immunization Branch in April, but never stated that she received a response. The relevant time period for the requested UIPA log is for requests made July 1, 2021 to June 30, 2022. Best, Brian
