Aloha, thank you for the note about OEQC. There were a couple of files that did not sufficiently identify the relevant agency. There was a file labeled as OPHP and one labeled as OMCCR; if you let me know which agencies those are, that may address two of the issues identified below. There was a file in a different format as well, which did not have a descriptive label to identify the agency ("UIPA Log 1.2022-6.2022.xlsx") and if identified may help with the issues below.

Hi Brian,
* OPHP is the Office of Public Health Preparedness
* OMCCR is the Office of Medical Cannabis Control and Regulation
* The State Hospital is included in the Adult Mental Health Division
* The Communications Office is included in the Director's Office (DO)
* The Environmental Management Division logs are all of the logs that start with EMD
* The Disease Investigation Branch is the unlabeled excel spreadsheet.
* The Family Health Services Division offices are the logs that begin with FHSD.
* The offices that did not include semi-annual logs reported that they did not receive UIPA requests for those time periods.
* I am confirming that HIPAA, HRO, OHCA, and WWB did not receive any UIPA requests for FY22 and will let you know as soon as I am able to confirm.

In prior years, State Hospital, Communication Office, EMD, and FHSD had their own logs, but understand if those got consolidated because the numbers tended to be low (if any at all).
Given historical numbers, most of this makes sense. But I would ask that you please check with Immunization Branch, Safe Drinking Water Branch, and HEER. It would be unusual if Immunization Branch had 0 requests all year. Given all the activity with Red Hill in the last year, I thought Safe Drinking Water Branch would have at least a few requests. And HEER typically has a relatively higher volume all year, so no requests in the first half of FY 2022 seems strange.
Lastly, I apologize; I missed that CAMHD is only the second half of FY 2022. Given its relative volume, it would be strange that it had no requests July - December 2021.
