Aloha Mary, thank you for sending that record. I would like the document in its original electronic format (not PDF); it appears that some of the information was cutoff in the PDF. Also, am I correct that the Department is not using OIP's form? The standard form for tracking requests is at https://oip.hawaii.gov/wp-content/upl.... If the Department has the standard OIP form completed for either the first or second half of FY2017, I would prefer that record. And I noted that all of the requests on this log occurred after January 1, 2017. Did the Department have any requests in the first half of FY 2017 (July 1, 2016 to December 31, 2016)? According to the information uploaded to data.hawaii.gov, the Department had at least one request during that period. You can see that data on Row 35 at https://data.hawaii.gov/Government-Wi.... Good Morning Mr. Black, Are you requesting only the request to access a government record form? Or are you also requesting the documents that were sent along with the request? Thanks, Mary

Aloha Mary, I don't need the actual request or related documents. I am only looking for those OIP forms for tracking UIPA requests (the Excel spreadsheets linked in my last e-mail). If the Department does not follow that format for tracking UIPA requests, then I am looking for whatever summary the Department uses to track requests. So for the second half of FY 2017 (January to June 2017), the document that you sent would work, but I need it in its original electronic format (Word, Excel, etc.), rather than as a PDF. For the first half of FY 2017 (July to December 2016), the document that you sent did not include any requests from that period, which leads me to think that there may be another summary document somewhere because the Department reported at least one request during that period. I am looking for that additional summary document in its original electronic format. You can call me at 531-4000 if it would help to chat through the request. Regards, Brian

Aloha Mary, OIP has so many forms. I was referring to the one posted at https://oip.hawaii.gov/wp-content/upl.... That Excel spreadsheet is a special format for tracking an agency's UIPA requests. The information that you sent works for what I need. Regards, Brian
