Dear Sir or Madam, The TMK for the main Naalehu LCC is (3)9-5-024:011 on Opukea Street. There are two other near-by LCCs. This well has been in existance for many years, first under C. Brewer and then the COH in 2010. The COH just raised fees for people for these LCCs and I was wondering if the DOH has registered these LCCs? Sincerely yours Sandra Demoruelle

Sandra There are no Safe Drinking Water Branch (SDWB) Underground Injection Control (UIC) permit or registration records associated with the project nor the property which you subsequently identified with TMK (3) 9-5-024:011. However, we have project review correspondence which I have attached. If I remember correctly, EPA was the lead regulatory agency for this project. Dear Sir, The COHDEM has, de facto, been using an UIC to dispose of their Naalehu sewage. Why haven't they had to register this with DOH and NPEDS? The number of laws COHDEM (in Naalehu alone) is violating is astounding. Sincerely yours Sandra Demoruelle
