All records in the possession, custody, or control of the Department of Planning and Permitting (DPP) relating to the following: 1. Notice of Violation No. 2022/NOV-10-051, issued October 13, 2022, concerning property at 5339 Kalanianaole Highway, Honolulu, Hawaii 96821. 2. Notice of Order No. 2023/NOO-858, issued September 15, 2023, concerning the same property. 3. All records indexed, filed, or stored under Tax Map Key 3-6-003: 042 (POID 152302) and/or Tax Map Key 3-6-003: 0427 (POID 152302), including but not limited to violation records, inspection records, permit records, correspondence, notes, and database entries. 4. All internal communications, emails, memoranda, or notes among DPP staff regarding the above NOV, NOO, or property, including communications involving Inspector Alfonso Caro and Code Compliance Officer Alan Tamanaha. 5. All database records, audit logs, change histories, or system entries reflecting when any fine, fee, or monetary assessment was entered, modified, or deleted in connection with NOO No. 2023/NOO-858 or the above TMK numbers, including the date and identity of the person making each entry. 6. The Declaratory Ruling issued on or about December 4, 2023, in connection with the appeal filed by Jewell Tuitele on behalf of Calvary by the Sea Lutheran Church regarding the above NOV. 7. All records relating to sign permit application and permit No. S2025-07-0021, including any DPP system records reflecting outstanding fines or fees searched or disclosed in connection with that application. 8. All records relating to any inspection of the above property conducted in 2025 or 2026, including any inspection related to a license application by Seagull Schools. 9. All records reflecting the correct Tax Map Key associated with POID 152302, and any records documenting how Tax Map Key 3-6-003: 0427 came to be associated with this parcel or these proceedings. Purpose of this REQUEST: Requested by counsel for Calvary by the Sea Lutheran Church (Rafael del Castillo, Rafael del Castillo & Associates, AAL, LLLC) in connection with a disputed civil fine.
