Summary of Request: We are requesting the most recent USDA Food & Nutrition Service form FNS-13, Annual Statement of Income and Expenditures for the Hawaii DOE School Food Services Branch. We would like to obtain completed FNS-13 forms from school year 2020-21, as well as the prior 5 years in order to compare changes in expenditures and revenue over time. Our organization is currently calculating the labor to food cost ratios for USDA child nutrition programs (CACFP, NSLP, SBP, SFSP) for Hawaii. The USDA has stated that the FNS-13 form is the most reliable source of information for computing the ratio of food and labor to total costs. The HIDOE School Food Services Branch is the largest School Food Authority in Hawaii. This data will assist Hawaii-based organizations--in particular those operating reimbursable meal programs for children during after school and summer programs--in advocating for a higher per-meal federal reimbursement, thus helping them serve more children.
