PE has been contracted to perform a Phase I Environmental Site Assessment for the property listed below: • 94-520 Kunia Road, Honolulu County, Parcel Number 920010190000 A Uniform Information Practices Act (UIPA) request is necessary in order to complete this assessment. We request access to copies of records pertaining to the following: 1) What is the current Fire Code enforced at the property? 2) What is the date of the last Fire Department Inspection (if applicable)? 3) Are there any current or outstanding Fire Code Violations existing against the property? If yes, please attach a copy of each violation. 4) Are there currently or have there ever been any hazardous material spills, complaints, above ground storage tanks (ASTs) or Underground Storage Tanks (USTs) on the property or in the vicinity of the property? 5) Is there any record of fire response and use of fire fighting foam at the property? 6) Have there ever been any other potential environmental concerns on or in the vicinity of the property? If there are any applicable fees for these records or research of these records, payment of fees must be approved prior to conducting.
