Summary of Request
I respectfully request copies of all non-conformance reports and/ or Corrective Action Reports that relate to a discrepancy of opinion in casework, a misidentification in casework, or an unexpected result in a proficiency test within the forensic laboratory’s Firearms and/or Firearms and Toolmarks Unit from January 1, 2015 to January 1, 2020. (I submitted a similar request before but those were for records between 2020 to 2026.) For purposes of this request, non-conformance reports/ Corrective Action Reports include records documenting deviations from standard operating procedures, quality-management system requirements, accreditation standards, proficiency testing requirements, or other established laboratory policies. This includes records describing the identification of the issue, any investigation, corrective action, or follow-up. To aid your search, these records are typically maintained by the laboratory’s Quality Assurance Unit, Accreditation Unit, or equivalent quality management function. If any portion of the requested records is withheld, please identify the specific statutory exemption relied upon and release all reasonably segregable portions of the records. Electronic copies are preferred. Please acknowledge receipt.
